The Long Term Influence of Discectomy on the Lumbar Segmental Notions


Abstract
This study was designed to evaluated the long term influences of the discectomy on each segmental motion, especially according to the discectomy level. The lumbar segmental motion after disctomy has remained controversial. A previous study has
shown
that the primary lumbar spinal motions were generally reduced after surgery. On the contrary the primary lumbar spinal motions were generally reduced after surgery. On the contrary an experimental study has presented that the motion significantly
increased on the level of discectomy.
Of the patients who underwent single level discectomy, twenty three patients were able to take flexion-extension radiographs. The mean follow up time was 4.2 years. the average age was 38.3-years-old. Twelve patients underwent L4-5 discectomy and
eleven
patients underwent L5-Sl. The primary segmental motions were measured and the proportional motion were calculated. Statistical analysis (t-test, ANOVA) was used to compare both groups. The motions of the segments including discectomy level were
generally reduced except in the L4-5 segments with L5-Sl discectomy. The segmental motions were significantly influenced by the discectomy level (P<0.05). In the L5-Sl discectomy group, the L4-5 motion was not reduced and significantly increased
comparing with the same level of the L4-5 discectomy group(P<0.05). The proportional motions of the L4-5 group were not different with the values from the normal. Those of L5-Sl group showed increased on the L4-5 segment(P<0.05).
These results suggest that preoperative evaluation for hypermobility or subclinical instabiliry of the L4-5 segment is needed before the L5-Sl discectoomy, since relative high segmental motion in this segment postoperatively may provoke symptoms.
